Notice of Application and Hearing

The following is a list of the items and forms you will need to proceed with a Review of a Co-decision-making order via a notice of application and hearing:

• Application: Form 6
• Affidavit of Applicant: Form 7
• Capacity Assessment Report if:
• • required by co-decision-making order
  • capacity is at issue
  • applicant is requesting termination of the order
  • if believed a significant change in capacity has occurred
• For continuation, amendment or replacement of co-decision-making order:
• • Consent—assisted person: Form 11
  • Consent- each proposed co-decision-maker (includes consent regarding criminal records check): Form 12
• Draft Order to which the assisted person has consented (optional): Form 10
• Notice of Application and Hearing: Form 9
• Personal References: Form 30
