• Affidavit of Applicant: Form 20
• Capacity Assessment Report if:
• • required by guardianship order
  • capacity is at issue
  • applicant is requesting termination of the order
  • if believed a significant change in capacity has occurred
• For continuation, amendment or replacement of guardianship order:
• • Guardianship plan: Form 32
  • Record of Decisions for each guardian: Form 33
  • Consent - each proposed guardian - (includes consent regarding criminal records check) - Form 24
  • Consent - each alternate guardian (includes consent regarding criminal records check) - Form 26
• for proposed new guardian or new alternate guardian
• • 2 personal references for each proposed new trustee or new alternate trustee- Form 30
• Draft Order (optional) – Form 23
