Fill in the required information, then upload and submit at least 1 document related to Seniors Financial Assistance programs.



Document requirements:

  • Only upload documents for one Personal Health Number per session.
  • If you belong to an organization and are uploading documents for multiple seniors at a time, ensure you start a new session for each individual. Failure to do so may result in processing delays.
  • Upload up to a maximum of 5 files per session for each individual. You will be prompted back to this page if you need to upload additional files.
  • You can upload documents in these file formats: PDF, JPEG, PNG, or TIFF
  • All files submitted at the same time can total 120 MB
  • Do not upload password protected or encrypted files. We are not able to process protected files.

How to submit dental and optical claims: Alberta Blue Cross administers benefits for the Government of Alberta’s Dental and Optical Assistance for Seniors programs. To submit your claim(s) to Alberta Blue Cross use an online account through Alberta Blue Cross' Member site or by mail to Alberta Blue Cross Health Services, 10009 108 Street NW, Edmonton, AB T5J 3C5.

Privacy: The personal information collected through the Alberta Seniors submit documents portal is for the purposes of administering the Alberta Seniors Benefit, Supplementary Accommodation Benefit, Special Needs Assistance for Seniors, and Dental and Optical Assistance for Seniors programs, and if applicable, the Seniors Home Adaptation and Repair Program and Seniors Property Tax Deferral Program. Information will be input into an automated system to generate content, make decisions, recommendations or predictions. This collection is authorized by sections 4(a) and (c) of the Protection of Privacy Act, the Seniors Benefit Act (RSA 2000) and section 2(1) of the Seniors Benefit Act General Regulation, the Seniors’ Home Adaptation and Repair Act and sections 3(1)(d) and 20 of the Seniors’ Home Adaptation and Repair Regulation, and the Seniors’ Property Tax Deferral Act and section 6(3) of the Seniors’ Property Tax Deferral Regulation.
